Date of Birth Not Required on Payroll Voucher Forms

Screen Actors Guild has been made aware that on several of the payroll companies’ new voucher forms there is a section that asks for members’ birth date, with some forms indicating that the payee’s date of birth must be provided in order to be paid.

Members do NOT need to provide date of birth information on these forms in order to be paid. The Guild advises that members cross out that section of the voucher—the company is still obligated to issue payment as long as the employee fulfilled the I-9 obligation.

Screen Actors Guild has been in contact with several companies to advise them that the Guild strongly objects to these newly issued vouchers requiring that SAG covered background performers insert their dates of birth and also strongly objects to making providing such information a condition of payment. The Guild has since been advised by one company that they will discontinue use of their new voucher and that a corrected version of the voucher will not include any language that indicates a date of birth as a requirement for payment. This company has acknowledged that background performers cannot be required to complete the birth date information.
